اجرا وتطبیق پروگرام های جدید انکشافی

پا بند بودن به وظیفه و رعایت قوانین وضع شده

سهم گیری درترینینگ ها و انتقال دانش آموخته شده به سایر کارمندان مرکزصحی مطابق به پلان تریننگ داخلی مرکزصحی

نظارت از فعالیت های کارکنان صحی جامعه و تمام فعالیت های صحی به سطح جامعه

مساعدت در تریننگ ها و تقویه ارتباطات میان کارکنان صحی جامعه و شورا های صحی

تقویه و گسترش بخشیدن همکاری میان جامعه ومرکزصحی

مسؤول شناخت وبیان مشکلات صحی جامعه

ارائه تعلمیات صحی برای اهالی ساحه تحت پوشش

بررسی وارزیابی اجرات کارمندان صحی جامعه

شناسایی ضروریات تریننگ کارکنان صحی جامعه

فراهم نمودن تریننگ های ریفریشر برای کارکنان صحی جامعه

فراهم نمودن تریننگ رهبری جامعه برای شورا های صحی جامعه

مساعدت در تجدید نقشه جامعه در مرکز صحی پوسته های صحی

مساعدت درساختن پلان های صحی در سطح مرکزصحی , شورای صحی وساحات تحت پوشش

تطبیق، نظارت وارزیابی از فعالیت های پروگرام صحی جامعه درساحات تحت پوشش مرکزصحی.

دریافت وگزارشدهی عاجل از هرنوع مشکلات در عرضه خدمات صحی به سطح جامعه

رهنمایی و حمایت کارکنان صحی جامعه درانکشاف وتطبیق پلان های کاری ایشان

تهیه پلان نظارتی ماهانه کارکنان صحی جامعه و شریک ساختن آن با مسول مرکز صحی

کسب اطمینان از اکمال منظم بسته مواد کارکنان صحی جامعه وتعویض بموقع مواد مصرفی نظر به ضرورت

تشویق نمودن کارهای گروپی درمیان CHW ها ، بخصوص هنگامیکه آنها در نفوس ساحه تحت پوشش فعالیت مینمایند

راپوردهی منظم از فعالیت های کارکنان صحی جامعه به مرکزصحی

بسیج نمودن جامعه جهت اشتراک در مسائل و پروگرام های صحی به سطح جامعه

بلند بردن آگاهی جامعه در مورد فعالیت های صحی در سطح پوسته های صحی و مرکز صحی

نظارت از کیفیت راجسترهای تصویری,تالی شیت تصویری ونقشه جامعه ومساعدت تخنیکی با کارکنان صحی جامعه

تکمیل راپور های MAR ازپوسته های صحی ساحه تحت پوشش

مساعدت با مسؤول مرکزصحی درتوحید گزارش های MAR در MAAR و استفاده ازگراف ها جهت بررسی پروگرام های صحی مرکزصحی

راپوردهی درست و بموقع از سایرفعالیت ها (راپور مصارف ادویه کارکنان صحی جامعه، راپور ربع وار بخش توبرکلوز و واقعات عاجل )

مساعدت در سروی های ضروری ایکه براه انداخته میشود

تشکیل کمیته های صحی جامعه (شورای صحی)

بلند بردن آگاهی شورای صحی دررابطه بهBPHS وموضوعات صحی مطروحه

جمع آوری فیدبک ازنحوه عرضه خدمات صحی مرکزصحی از سطح جامعه و رضایت مراجعین به آمرمرکزصحی

تقویت بخشیدن سیستم رجعت دهی میان جامعه، پوسته های صحی و مراکز صحی و بر عکس آن

اجرای وظایف سپرده شده از طرف آمر کلینیک

سهم گیری فعال درتطبیق ستندرد های جدید بهبود کیفیت

Job Requirements:

Education: Bachelor of Social Science, Diploma in Nursing or Pharmacy and bachelor’s in public health is preferred

Experience:

2 years of work experience in health and community health Program

Skills:

Good communication skills in Dari and proven management ability.

Languages:

Written and Spoken fluency in local languages (Dari) is a must.

General:

Local resident of Bamiyan Province, especially from the covered area of HFs (Kawshaw BHC) is preferred.
