Deliver timely and appropriate case management by evaluating the needs of individuals with protection concerns, creating and executing customized case plans, and ensuring proper follow-up while fostering a trusting and supportive environment.

- Conduct intake interviews with individuals identified by the outreach team or referred by other organizations, and open cases if vulnerability criteria are met.
- Perform thorough assessments for each client with their consent, taking into account emotional, behavioral, and social factors that impact their psychosocial well-being, as well as protection and risk factors.
- Conduct home visits to evaluate clients' living conditions as needed.
- Develop a personalized case plan with the client based on the needs identified during the assessment.
- Refer clients to appropriate services and, when necessary, accompany them to various service providers.
- Provide ongoing basic emotional support and counseling through regular follow-up meetings, building a relationship founded on trust and maintaining confidentiality.
- Document cases on the internal case management platform, ensuring that progress on the action plan and significant changes in the client’s psychosocial well-being and overall situation are recorded promptly.
- Ensure proper and secure filing and archiving of case management files to protect data confidentiality.
- Close cases when the objectives of the case plan are achieved and no further needs are identified.
- Carry out all tasks in accordance with key protection principles, including do no harm, non-discrimination, confidentiality, accountability, and need-to-know principles.
- Participate in internal case management meetings and promptly notify the Case Worker Team Leader of any cases with special or emergency needs.
- Maintain good relationships and coordinate with main service providers in the intervention area to enhance the identification of individuals with specific needs for timely referral, follow-up, and information sharing.
- Identify and report gaps in the availability and quality of service provision for clients in the target areas.
Education
- A university degree in a related field such as Social Work, Psychology, or a similar area is advantageous.

- Basic computer literacy (Word, Excel, and internet).
