Job Description:

POSITION SUMMARY:
The Community Health Worker (CHW) is a volunteer position that manages health posts, each
health post has two CHWs, (Male and female), and their prime responsibility is to help patients and
their families navigate and access community services, and other resources, and adopt healthy
behaviors. The CHW supports providers and the Case Managers through an integrated approach to
care management and community outreach. As a priority, activities will promote, maintain, and
improve the health of patients and their families. CHW provides social support and informal
counseling, advocates for individuals and community health needs, and provides services such as
basic first aid, blood pressure screenings, and simple wound care.
• KEY RELATIONS
• Reports to the Community Health Supervisor and Health Committee.
• Collaborates with Members of the Basic Health Centre (BHC) and the ECHO Project staff.
• Actively contributes to and supports the Basic Health Centre and outreach team by initiating
awareness sessions and facilitating referrals of cases to health facilities.
2. Summary of Key Functions
• Build trusting relationships with patients and their families, offering general support and
encouragement.
• Provide ongoing follow-up, utilizing basic motivational interviewing techniques and
assisting with goal setting for patients and their families.
• Conduct follow-up activities such as phone calls, home visits, and visits to other settings
where patients can be found.
• Assist patients in setting personal health-related goals and ensure they attend appointments.
• Healthy Education and popularizing healthy habits.
• Referral of patients to the health center (referral) whenever needed
• First aid
• Treatment of common and simple diseases such as treatment of respiratory diseases,
diarrhea, and malaria) according to the guidelines of the Comprehensive management of
Childhood Diseases at the community level

Follow-up treatment of tuberculosis patients under direct observation

Maintain excellent working relationships with patients, visitors, and staff, effectively communicating HHC's mission.

Collaborate closely with medical providers to ensure patients have comprehensive and coordinated care plans.

Act as a patient advocate and facilitate communication between the patient/family and community service agencies.

Document patient care management information in the EMR and other software within 24 hours of patient contact.

Attend regular staff meetings, trainings, and other requested meetings. 3. General Responsibility

Actively participate in significant community events.

Encourage and persuade families and the community to participate in vaccinating children and women of reproductive age.

Support national decisions and measures at the village level, actively engaging in all campaigns.

Conduct awareness sessions within the community, educating them about the harmful effects of drugs.

Promote healthy feeding habits, emphasize early breastfeeding and exclusive breastfeeding for the first six months of life, advocate for balanced nutrition, and encourage the use of iodized salt by families.

Explain the importance of prenatal and postnatal care to society, encouraging regular and timely utilization of healthcare services.

Encourage mothers to engage skilled birth attendants and assist families in preparing birth plans.

Educate family members about identifying danger signs of pregnancy and childbirth complications and collaborate with them to prepare for prompt medical attention.

Promote the uptake of family planning services among couples and encourage interested families to consider long-term family planning methods.

Provide first aid services for common accidents at the family and community level, adhering to a specified list of qualifications and skills.

Have knowledge of community resources suitable for the needs of patients and their families.
3. Management responsibility
• CHWs selected and HPs established in the surrounding areas of the Health Facility.
• Local community awareness level improved regarding active participation in organizing mass campaigns and vaccination program.
• Community level referral of increased to Health Facility.

4. COMPETENCE, SKILLS AND EXPERIENCE

Being able to Familiarize himself/herself with community members and plan his/her work area to provide health services effectively.

Collaborate with the local council to include and report births and deaths of mothers, infants, and children up to five years of age, utilizing the health department's reporting form.

Ensure proper management of supplies and report on the distribution of items such as medicines and accessories provided to community health workers.

Raise awareness in the community about common eye and skin diseases.

Establish a family support group to facilitate behavior change and promote the utilization of health services.

Aim to cover 200-300 families as a community health worker.

Conduct monthly surveys using the Babies form and perform biannual surveys as part of the maternal and child surveillance system.

Provide door-to-door health information to the community, focusing on prenatal, birth, and postnatal care, family planning, and encouraging women to consistently access maternal and child health services in a timely manner.

4.2 Skills and Experience:

Good organization, planning, and reporting skills.

Fluent in the local language, a good understanding of the culture

Willingness to learn new skills, through active participation in the training.

Education and working experience.
Secondary education and Graduation from 12 grade will be preferred. with at least one year of previous experience in the relevant field and/or working experience in the community.
4.4 Languages Skills:

Fluency in spoken and written Dari and Pashtu is a requirement. English Language skills are a plus point
